In the case that you are more than 15 minutes late to your child's appointment, you will be asked to reschedule.
If you are unable to make your child's appointment and do not show up without cancelling or rescheduling, we will charge a NO SHOW fee of $25. After 3 no shows, we reserve the right to discontinue care.
While we will do all that is possible to accommodate, the first available appointment may or may not be on the day that the original appointment was missed. Ultimately, it is your responsibility to call our office if you are unable to show up at your scheduled time.
Our office follows the vaccination guidelines set by the CDC and the American Academy of Pediatrics. All patients in this practice are REQUIRED to receive all recommended doses of vaccines. If you absolutely refuse to vaccinate your child, we recommend that you find another health care provider who shares your views. We no longer accept new patients to the practice who “refuse to vaccinate”. We reserve the right to discontinue care with any established patient that refuses to follow the proper vaccine schedule.
Our office requires 3 to 5 business days for forms to be completed. During heavy form season (June through November) the turnaround time required for forms to be completed is 7 to 10 business days.
Please notify our staff in a timely manner if you need a form urgently, same day requests may not be fulfilled.
If your child does not have insurance at the time of their visit, you will be required to pay for said visit. These out-of-pocket payments are non-refundable even if your child receives insurance at a later date.
For new patients or those who have inactive insurance on our end are required to provide a copy of their insurance. If insurance is not provided at the time of the appointment, you will either be required to pay out of pocket or rescheduled to when an active insurance can be provided.
It is very important that you call your insurance company before your scheduled visit to confirm whether or not we are in your network. If you are changing your insurance plan, we urge that you verify if our physician is listed as a provider within the networks that you are considering.
You are responsible for knowing what is and is not covered under your insurance plan.
